Since its 3.1 release, the Psychostats tools can generate heatmaps (as this one).

This archive provides all you need for all the official de_ maps (de_aztec, de_cbble, de_chateau, de_dust, de_dust 2, de_inferno, de_nuke, de_piranesi, de_port, de_prodigy, de_tides and de_train)
